Job: Physical Therapist

The Physical Therapist is a professional member of the home health team who is responsible for providing physical therapy to patients at home in accordance with Agency policies and the plan of treatment. The registered Physical Therapist is also responsible for the supervision of the Physical Therapist Assistant.

RESPONSIBILITIES:
The Physical Therapist may perform admits, resume care, recertify, discharge and visits throughout a case as necessary according to the policies, procedures and regulations of Fusion Healthcare.

Provides physical therapy services as prescribed by the physician which can be safely provided in the home.

Assist the physician in evaluating patients by applying diagnostic and prognostic muscle, nerve, joint, and functional abilities.

Provides physical therapy services and directs and supervises physical therapy by the Physical Therapist Assistant.

Treats patients to relieve pain, develop or restore function and maintain maximum performance, using physical means, such as exercise, massage, heat, water, light, and electricity.

Arranges for provision on an outpatient basis or services listed above which cannot be given in the patient's home.

Observes, records patient activities and findings; submits documentation in a timely manner for inclusion in the clinical record.

Observes, records, and reports to the physician and/or clinical supervisor the patinet's reaction to the treatment and any changed in the patient;s condition.

Instructs the family in care and use of equipment.

Instructs other health team personnel including, when appropriate, Home Health Aides and family members in certain phases of physical therapy with which they may work with the patient.

Instructs the family on total physical therapy program.

Develops a patient care plan and measures progress or lack of progress in goal achievement.

The Physical Therapist must be licensed in the state of Florida and shall have minimum of one year of experience in Physical Therapy.
