Palliative Care Manager
Job ID#: 1060 Location: Clearwater, FL - Roosevelt Community Service Center Functional Area: Clinical - Other Management Department: Palliative Care Position Type: Full-Time Regular Education Required: Associates Degree Experience Required: 3 - 5 Years Relocation Provided: No At Suncoast Hospice, the Palliative Care Manager utilizes management skills and expert knowledge of state and CMS home health regulations to facilitate provision of care in Homecare and Suncoast Supportive Care. Under physician's orders, per the Physician's Plan of Treatment, the manager is responsible for identifying and coordinating patient/family needs and for providing palliative, preventative, and rehabilitative care to the patient/family unit. The manager is a mentor, teacher, and preceptor to new staff and team members. The manager coordinates and participates in quality management initiatives and financial management of the care programs. The manager is responsible for agency adherence to home health conditions of participation and state regulations. The Palliative Care Manager participates in agency's strategic initiatives to increase access to the care programs. 1. Current Florida Registered Nurse license, BSN preferred.
2. Minimun of three (3) years nursing experience; with at least one (1) year acute care setting. 3. Minimum of two (2) years home health experience with thorough understanding of State and Federal Home Health Agency Regulatory Standards. 4. Minimum of two (2) years home health management experience with supervisory responsibilities 5. Interest, experience and expertise in the care of patients with chronic and/or life limiting illnesses. 6. Must have competent computer skills. This job posting is no longer available on Jobing.com.
