Job: Case Manager

JOB FUNCTION: Functions as a member of a multi-disciplinary team in providing case management services such as referral, linkage, advocacy, monitoring, and assessment for assigned caseload.

QUALIFICATIONS: Bachelor's degree from an accredited university with major course work in the areas of psychology, social work, health education or a related human services field and a minimum of one (1) year of full time or equivalent experience working with children who have serious emotional disturbances; or Bachelor's degree from an accredited university and at least three (3) years full time or equivalent experience working with children who have serious emotional disturbances.

PHYSICAL REQUIREMENTS: Ability to hear and see in emergency situations. Must be able to complete NAPPI training and perform NAPPI techniques. Ability to communicate orally and in writing.

SPECIAL WORKING CONDITIONS: Exposure to consumers and a variety of program conditions, which may include long, irregular hours, on-call duties, and local travel. Exposure to a wide range of community environments outside the agency.
