Job Description

Carries out casemanagement responsibilities for identified Veterans facing homelessness oreviction. Serves as liaison to affiliated GPD partners to provide neededassistance.

Application to Practice - Effectively matches client needs toappropriate levels and modalities of care; individualizes strategies andinterventions to individual client recovery needs, preferences, readiness,stage of change and unique co-occurring disorder, language, gender and culturalconsiderations; makes optimum therapeutic use of client crises; incorporatesand utilizes pharmaceutic interventions as appropriate; adheres to the fidelityof selected practice models.

ProfessionalReadiness - Understands and respects diversecultures and cultural differences; comfortable with and responds to personswith disabilities; evidences personal awareness of strengths, limitations andbiases; behaves ethically and adheres to professional standards ofconduct; pursues guidance andsupervision and continuous professional development opportunities; remainsaware of personal and professional limits in knowledge and expertise.

TechnicalCapabilities - Proficient in the use of Electronic Health Records; effectively utilizeselectronic medium (internet, intranet, phones, etc.) to communicate and stayabreast of changes in the field and political and community environments;thoroughly documents and records professional events, activities andtransactions.

BusinessAcumen - Knowledgeable of support and benefits available to clientsand aggressively works to enable client access to those resources; proficientin the matching and billing of services provided to appropriate fund sourcesand maintaining audit compliant backup documentation; consistently meets orexceeds established case load size and productivity standards.

Professional- Behaves ethically; presents a professional image in appearance,communications and demeanor; adheres to agency scheduling policy; followsprogram activity schedules; committed to personal professional development.

Analytical- Establishes a helping relationship with the person receiving services bydemonstrating empathy, respect, and genuineness in order to build trust; assistsin gathering information about basic and community living of the personreceiving services to be used in the assessment process.

Leadership - Seeks to participate in effective approaches and training to enhance jobskills; seek opportunities to mentor new staff; ability to recognize unsafebehaviors, situations, and environments to maintain the safety and well-beingof persons receiving services, staff and community members.

Pre-requisite Requirements:

A Bachelors' degree in human services,administration, management or related field.

Twoyears of experience relative to the responsibilities listed (housing, casemanagement, accounting, contracts management or other related experience maysubstitute on a year for year basis for the above degree).

Supervision: Work is performedunder the direction of the Program Director and does not provide supervision toothers.

Scope of Responsibility: Carry out casemanagement for eligible Veterans in Pasco, Hernandez and Polk Counties. Keepaware of needed changes, provide feedback from the field, and stay abreast ofcurrent/new information to support duties.

CoreResponsibilities/Essential Functions:

Completeadmission and orientation documentation; may administer or interpret screening/assessmentinstruments and treatment plan.

Developplans, which include services to accomplish employment , increased income, andaccess to permanent housing

Documentall clients' contacts, care and response, maintain records and updated rosters collectdata and prepare reports as mandated by program procedure within agencystandards and timeframes.

Coordinateservices with the Veterans Administration as needed and facilitatecommunication with Grant Per Diem contract manager.

Maintaina comprehensive listing of community resources, including social, legal,housing and educational providers.

Attendscheduled interdisciplinary treatment team meeting or clinical supervisorysessions and discusses aspects of client's care and progress (or lack of same);may participate in other meetings/events as assigned.

Recognizeand promptly/appropriately assist as needed in crisis situations to minimizeincidents of hospitalization or incarceration; report treatment needs ofclients to clinical personnel and supervisor; recognize client's comprehensiveneeds and discusses with interdisciplinary treatment team.

Completedocumentation including but not limited to behavioral progress reports,incident reports, grievances, injury reports, client eligibility, enrollment,tracking, and lease agreements, billing and related documentation as required.

Performrelated work as requested / required.

Maintainsfull knowledge of all GPD grant procedures and requirements; remain currentwith any/all changes related to GPD.
