Case Manager/Data Specialist, CTC, Sarasota
Position Summary: Ensures continuity of care to clients through case management services, specifically in mental health and co-occurring disorders. Links clients and their families to needed resources to help improve the quality of life for the client. Assists in data collection on client outcomes and grant reporting.
Minimum Qualifications: Bachelor's Degree in Social Work, Psychology, or related field. Also requires knowledge of community services. Strong communication, ability to work on a team and time management skills necessary. Valid Florida driver's license, current auto insurance and a good driving record.
Preferred Qualifications: Two years' experience in a mental health setting and
previous case management experience preferred.

Essential Functions/Job Duties:
Perform intakes and orient new clients to the program.
Provide direct services to clients to teach behavioral symptoms and management techniques and to promote personal growth.
Links clients with needed services and resources.
Act as liaison and consult with community agencies and judicial system to maintain coordination in the treatment process.
Document daily client progress in treatment.
Provide ongoing assessments of client's mental status; e.g. symptoms and response to treatment.
Participate with team in providing treatment, education and services in the areas of employment, activities of daily living, leisure time, housing, substance use, symptom-behavioral management and coping with stress.
Provide crisis intervention services.
Track client progress for grant reporting requirements of both active and discharged clients.
Work Environment/Physical Demands: This position operates in both an office environment and community based settings. Duties of this job are often provided in the place where the client resides or needs the service. This position may be subject to exposure to individuals who engage in maladaptive behavior.
Position Type, Expected Hours of Work and Travel: This is a full time, 40 hour position. Typical hours of work are Monday through Friday- days. This position requires flexibility in work hours to include some evening and weekend hours as needed to meet the needs of the clients. Frequent local travel is required. On call rotation is required.
